In order to protect the health and safety of employees, staff, and students, all those who access Pasco County School locations must self-screen for symptoms and other risk factors of COVID-19 daily. If you answer YES to any of the questions, you should NOT be on campus. Access the Visitor Screener or the Student Screener online. Employees should use the Employee Self-Screener.

USF Happiness

There are three ways to return the permission form for the happiness screening:

  1. The blue permission form was sent home with your student on Wednesday, September 9th.  You can review, sign, and return the form with your student. They will drop off the form at the cafeteria, and receive a small thank you candy from a school counselor. If you don’t want your child to take part in the screening, simply write that on the bottom of the form and your child will still receive the treat. 
  2. We also attached an electronic copy of the permission form for screening to the Connect Ed form. You can review, sign, and send the form to Ms. Foti mbova@pasco.k12.fl.us or to USF at kshum@usf.edu electronically. 
  3. If you would like to sign the form electronically through DocuSign, you can go to tinyurl.com/happyscreening. You will receive a link from Kai Zhuang Shum via DocuSign to review and sign the permission form for screening electronically. No printing or apps required.
